Booking Form






Please only fill in this form once you have ascertained that I am available for your wedding day.

Please read carefully and then complete this form. I will then contact you to make an appointment to discuss the ceremony and make sure that all the documentation is in place.

Legalities:

NB # As of August 2010, you will need to provide three passport-size photographs of the bride and the groom. Thumbprints of the bride and groom are now also required to be on the marriage register, but I take care of this.

1. A Form Bl 32A is required if either of you is under the age of 21.
2. If the Bride is younger than 15 and/or the Bridegroom is younger than 18, permission for the marriage is required from The Commissioner of Child Welfare.
3. A Form Bl 31 must be completed if either of you does not have an SA ID Document. (I have these).
4. If either of you is divorced, a clear copy of the final decree of divorce is required.
5. If either of you is a widower/widow, a clear copy of the death certificate is required.
6. In all cases, clear copies of both of your SA IDs or foreign Passports are required.
7. In all cases, clear copies of both of your witnesses SA IDs or foreign Passports are required.

I will need all this documentation at least one week BEFORE the date of your wedding.

The costs involved :
Ceremony : See individual ceremony details.
Marriage certificate and registration of marriage : No Charge.
"Unabridged" Marriage certificate (for overseas registration) : R400.00.
A 50% deposit is required to confirm the booking, and the balance no later than one week before the wedding. Please note that the booking will not be confirmed before the deposit is paid.

Round Trip Travelling Fee (from Tokai, Cape Town.) Includes fuel, vehicle wear and tear and my time.
0-20km : No charge.
21-40km : R100.00.
41-60km : R200.00.
61-80km : R300.00.
81-100km : R400.00.
More than 100km : Negotiable.

For example:
Hout Bay : R100.00
Sea Point, Table Mountain : R200.00.
Stellenbosch, Paarl, Franschhoek, Wellington : R500.00
Yzerfontein : R600.00

NB : Deposits are non-refundable in the event of cancellation.

In terms of section 26 (1) of the Birth and Death Registration Act, as amended, a woman after her marriage may:
(a) Assume her husband's surname;
(b) Resume a surname, which she bore at any prior time,
(c) Join her maiden surname or previous married surname with that of her husband's, or
(d) Retain her Maiden name.
